Prefect Application help please!!!?

hello, i am in year 10, and out head of year miss tanger has told us to make a prefect application, HEAD PREFECT, so obviosuly i wanna be a heaad prefect the thing is my head is blank of ideas, like i am a two times shot put champion in my town burnley, this was the year i came first again, i always come to revision classes, and i mean always, i have never been late, i do have certificates and stuff, for athletics, maths, tennis etc and heliping out and stuff, and i am already a prefect this year, i m in a sense in charge on fridays cos i am like the head on fridays so, could you please help with me writing my application, like what should i include, i m gonna make it on computer... help with like how to start it off, i mean like high english words and how i could end it thanks :)
